Typically, we can assess new patients within 3 business days of referral. For patients needing immediate treatment, we can often evaluate and treat the patient in one visit. You (or your patients) may also call
(781) 647-PAIN (7246) to speak with a Patient Care Coordinator who can schedule an evaluation.

Our initial comprehensive evaluation lasts 2 hours and includes:

• Medical Evaluation
• Detailed Functional Assessment
• Behavioral Screening
• Sleep Screening and Evaluation

We summarize this initial visit with a thorough   Interdisciplinary Assessment Report
that we send back to you.

Please download our Physicians Order Form and complete as much as possible. Our staff will then call the patient and schedule their appointment.
